About the Survey

What is the purpose of this survey?

We want to understand the mental health and wellbeing needs of our local community. Your feedback will directly influence how the council allocates funding, designs local public health initiatives, and improves community support groups, crisis services, and local amenities.

Who is eligible to participate?

Any resident currently living within the local council boundaries who is 16 years of age or older is encouraged to take part. We want to hear from people of all backgrounds, whether you have personal experience with mental health challenges or not.

How long will the questionnaire take?

The survey should take approximately 10 to 15 minutes to complete.

Privacy and Data Security

Is my data safe and anonymous?

Yes, your participation is completely anonymous and confidential. We do not collect identifiable personal information such as your name, exact address, or date of birth.

All responses are aggregated into data reports.

No individual can be identified from the published results.

All data is processed in strict accordance with the UK GDPR and the Data Protection Act 2018.

Why are you asking demographic questions?

At the end of the survey, we ask optional questions about your age group, ethnicity, and general neighbourhood. We collect this strictly to ensure our data represents our entire community fairly and to identify if specific demographic groups require more targeted local support.

Completing the Questionnaire

What kind of questions will I be asked?

The survey focuses primarily on the environmental conditions and support systems that influence your everyday wellbeing. You will be asked about:

Your general life satisfaction, anxiety levels, and feelings of loneliness over recent weeks.

Your awareness and usage of local council facilities (such as parks, libraries, and community centres).

Your ease of access to local mental health support, support groups, or advice services.

Do I have to answer every single question?

No. While complete answers give us the most accurate data, every question is optional. If a question makes you feel uncomfortable or does not apply to you, you can simply skip it and move to the next page.

Can someone help me fill out the form?

Yes. If you need a friend, family member, or support worker to assist you with reading or typing your answers, they may do so. We also offer alternate formats including Large Print, Easy Read, and translation services upon request by contacting the council’s support line.

Next Steps and Impact

What will happen to the results of this survey?

Once the survey closes, the data will be independently analysed to produce a Community Mental Health Needs Assessment. This report will be presented to the council cabinet to inform our public health strategy and budget allocations for the upcoming financial year. A summary of the final report will be published openly on the council website.

Will completing this survey provide me with direct medical help?

No, this survey is an anonymous data-gathering tool and is not monitored by medical professionals. If you are currently struggling or need immediate mental health support, please review the resources below.
